PCP promotes community life that honors the individual's preferences, choices, and abilities. The person-centered planning process involves families, friends, and professionals as the individual desires or requires.
Pre-planning is a key component of the person-centered planning process. During the pre-planning meeting the individual has the opportunity to identify their dreams and goals for the upcoming year. They have the opportunity to determine who they want to invite to the planning meeting to help support the achievement of those goals. The individual chooses where and when the meeting occurs and who is responsible for facilitating and recording the meeting.
The goal of person-centered planning is to support the individual to create a plan that prioritizes their needs, preferences and values. It acknowledges that they are the experts in their own life and what they want to achieve. It gives the individual the opportunity to have control over their own life.
